めまい外来 (庄司,小林)

メニエール病、良性発作性頭位めまい症、突発性難聴、前庭神経炎などの診断をつけられたり、原因不明で診断がつられずに、めまい、難聴、耳鳴などが持続したり、発作性に繰り返されている患者様が対象です。
脳や脊髄の障害でないなどを問診や診察や検査で区別します。内耳の神経節の神経細胞の核に、水痘帯状疱疹ウイルスが休眠していて、免疫能が低下する度に再活性化することが考えられ、20年以上前から抗ヘルペスウイルス薬の治療で良好な成績が報告されています。
当院では脳神経内科と耳鼻科が共同でこの外来を行っております。外来は月曜(9:00-12:00,15:00-17:30)、水曜(9:00-12:00, 15:00-17:30)、木曜(9:00-12:00)、金曜(9:00-14:00)、土曜(9:00-13:00)です。担当医は脳神経内科:庄司進一、耳鼻科:小林潔子です。桔梗ケ原病院の電話(0263-54-0012)で上記時間帯に当外来で、又は火・金曜の15:00-17:00に医事課で予約してください。

【Out-Patient-Clinic for Chronic Vertigo,Dizziness,Hearing-loss and/or Tinnitus(Shoji,Kobayashi) 】

The object of this out-patient-clinic are patients with diagnoses of Ménière’s disease, benign paroxysmal positional vertigo, sudden deafness, vestibular neuritis and patients with chronic vertigo, dizziness, hearing-loss, and/or tinnitus of unknown cause. By asking a patient detailed questions about his /her symptoms, a medical examination and laboratory examinations, we distinguish the cause of the patient’s symptoms from the disorders of the brain or the spinal cord from the disorders of the internal ear. There have been a hypothesis that chronic vertigo, dizziness, hearing-loss, and/or tinnitus may be due to reactivation of dormant varicella-zoster virus in the vestibular nucleus and/or the spiral ganglion neuron (both are groups of nerve cell in the internal ear) which may be precipitated by weakened immune system. Since more than 20 years ago, there were reports that antiherpes virus drugs were very effective to chronic vertigo, dizziness, hearing-loss and/or tinnitus.
Kikyogahara hospital opened this out-patient-clinic in neurology and otolaryngology jointly.
Neurologist is Dr. Shinichi Shoji, and Otolaryngologist is Dr. Kiyoko Kobayashi. This outpatient-clinic opens on Monday (9-12, 15-17:30), Wednesday (9-12, 15-17:30), Thursday (9-12), Friday (9-14), and Saturday (9-13). You have to make an appointment for an interview in advance by calling up (0263-54-0012) this out-patient-clinic at the timing described above or hospital office on Tuesday, Friday at 15:00-17:00.

物忘れ外来 (庄司)

物忘れと認知症とは生理的と病的の違いがあります。この区別は難しいことが多いのですが、物忘れ外来はこの区別をはっきりさせます。物忘れの程度をテストし、脳の画像を撮影し、これらの結果と症状の経過やその患者様の他の疾患やどんな薬を飲んでいるかなどと共に考えて、診断します。
生理的な物忘れであるとの結論なら、これからの生活の指導と、将来再び検査する必要があるかどうかや必要 な場合はどんな時期かなどがアドバイスされます。薬による物忘れである時は、どの薬が原因の可能性がある かをお話しし、その薬をやめることができるかを検討してもらえるように、その薬を処方している医師へ手紙 を書きます。内科的疾患や脳外科的疾患が物忘れの原因である場合には、その治療が受けられるようにアレン ジします。
脳の病気で認知症であれば、どの種類の認知症で、これからの経過をお話しし、その経過を良くするための生 活指導と薬のお話をします。主治医への手紙やこの外来で経過を診ることを希望される場合には、外来に通っ てもらい経過を診ていき適時適切なアドバイスを行います。

【Out-Patient-Clinic for forgetfulness(Shoji)】

Memory impairment and dementia are different conditions, the former is physiologic and the latter is pathologic condition. This distinction is frequently difficult. This Out-Patient-Clinic for forgetfulness can clearly distinguish between these conditions. Using memory tests and brain imaging including background diseases and history of drug intake of the patient this diagnosis can be done. When the forgetfulness has diagnosed as a physiologic condition, the patient can receive advice of the future life and the need and timing of re-examination of this symptom in future. The forgetfulness is diagnosed as a result of taking causing medicine, the patient can receive a letter to a doctor who prescribes the drug, asking the possibility of stopping it. When the memory impairment caused by medical diseases or brain surgical diseases, the patient can receive appropriate arrangement that he/she can get a suitable therapy to this disease. When the patient gets diagnosis of dementia caused by degenerative brain diseases, the patient can hear the detailed diagnosis and the future development and lifestyles and medicines for better course. If the patient chose the following up under the present doctor in charge, a letter to the doctor will be arranged. If the patient and family chose the follow up under this Out-Patient-Clinic,the patient can go to and from this clinic, and he/she can receive appropriate advice at the right moment.
